SHBG: What It Means for Your Testosterone Evaluation

Renewed Drive TRT explains SHBG in plain language — what it measures, why it may come up in a testosterone evaluation, and how to discuss it in consultation.

What Is SHBG?

SHBG stands for Sex Hormone-Binding Globulin. It's a protein made mostly by your liver that attaches to testosterone and other sex hormones as they circulate in your bloodstream. When testosterone is bound to SHBG, it isn't readily available for your body's cells to use. Testosterone that isn't bound — often called free testosterone — is the portion your tissues can actually put to work.

Because SHBG affects how much testosterone is biologically active at any given time, it's a value that sometimes comes up alongside total testosterone results. Two people can have similar total testosterone numbers but very different amounts of usable, free testosterone depending on their SHBG level.

Why SHBG May Matter in a Testosterone Conversation

SHBG levels can vary from person to person for a range of reasons, including age, body composition, thyroid function, liver health, and other individual factors. A higher SHBG level can mean more testosterone is bound up and less is freely available, while a lower SHBG level can mean more testosterone is circulating in its active, usable form.

This is one reason a single total testosterone number doesn't always tell the full story. Understanding SHBG can help explain why someone's symptoms and their testosterone reading don't always seem to match up on paper — a nuance that's worth exploring with a clinician rather than trying to interpret alone.

How SHBG Fits Into a Broader Picture

SHBG is typically one piece of a larger picture rather than a standalone answer. It's often considered together with total and free testosterone, and sometimes other related values, to get a clearer sense of what's happening hormonally. No single number determines whether testosterone therapy would be appropriate for a given person — that kind of individual review is something a clinician works through directly with you.

If you're trying to make sense of your own numbers or symptoms, it helps to bring your specific questions into a consultation rather than trying to self-diagnose from a lab report alone.

Where SHBG Fits Into the Testosterone Therapy Path

SHBG is one of several values that may be part of a broader conversation during evaluation. If you want to understand how lab discussions generally fit into the testosterone therapy process, the Evaluation & Lab Review page walks through what that stage typically involves. You can also browse the full Glossary for related terms you may encounter.

FAQs

Is SHBG the same as testosterone?

No. SHBG is a separate protein that binds to testosterone in your bloodstream. It affects how much testosterone is freely available for your body to use, but it isn't a measure of testosterone itself.

What can cause SHBG to be higher or lower than typical?

SHBG can shift due to factors like age, liver function, thyroid activity, and body composition, among others. Because there are multiple possible influences, it's a topic worth discussing individually with a clinician rather than assuming a single cause.

Will I automatically be tested for SHBG?

Whether any specific lab value is reviewed depends on your individual situation and a clinician's judgment. This page is educational and doesn't set or guarantee a testing protocol for every reader.

Does a high or low SHBG level mean I need testosterone therapy?

Not on its own. SHBG is one data point among several that a clinician may consider. Individual suitability for testosterone therapy is something only a clinician can determine after reviewing your full picture.

How does SHBG relate to free testosterone?

Free testosterone is the portion not bound to SHBG (or other binding proteins), meaning it's available for your tissues to use. SHBG levels directly influence how much free testosterone is present relative to your total testosterone.

Can I interpret my own SHBG result without a clinician?

You can learn the general meaning of the term here, but interpreting your specific result in the context of your health, symptoms, and other lab values is best done with a clinician during a consultation.

Where should I bring questions about my SHBG result?

Bring the specific number and any related lab values into a consultation. A clinician can walk through what it may mean for you personally and what, if anything, might be worth discussing further.
